I have a fruit set at about 12 feet out. There is a leaf stem right against the side of the fruit. Should I whack it?

Yep.
Before I even set fruit I will trim off the tendril, the leaf or secondary (in that order) if it's in the way of the fruit landing...
Anything that touches the fruit will scar it.
